Existing Patients click the Existing Patients hyper link below or Book Now button to set up a routine visit.
New Patients
Once your visit is scheduled you will receive an email with access to our secure patient portal in order to complete your new patient intake paperwork.
It is not overwhelming, can be completed in a matter of minutes and is essential for us to know what's going on with you to best serve you. Please have completed before your visit.
Existing Patients
Use the patient portal to manage your appointment schedule with us. If the appointment request or reschedule request is less than 12 hours please call us at (218) 451-4516.
